Caring for populations Essay

Introduction A community health nurse must be effective in targeting the population as a whole. The nurse will determine the needs for the community by using the data compiled from a windshield survey. As stated in the text, â€Å"A population focus also involves a scientific approach to community health nursing: an assessment of the community or population is necessary and basic to planning, intervention, and evaluation for the individual, family, aggregate, and population levels† (Nies & McEwen, 2011). The purpose of this paper is to establish a priority health problem of the community of Harlingen, based on demographic, epidemiological data and a windshield survey of the area. Community The community assessment for this project is Harlingen, Texas. According to the 2010 United States Census Bureau, its population is about 64,918 of which 47.8% are male and 52.2% are female. The density is approximately 1629.1 residents per square mile. The racial makeup is 1.0% Black or African American, 79.5% Hispanic or Latino, 18.0% White, 1.9% from two or more races, 1.3% Asian, 0.1% Native American. The median household income, 2008-2012 was $34,096 in Harlingen, compared to $51,563 in the state of Texas. The percentage of persons below poverty level from 2008-2012 was 33.2% in Harlingen, compared to 17.4% in Texas. The city of Harlingen is in the center of the Rio Grande Valley. It is a community which has a transient population of â€Å"Winter Texans.† These Winter Texans are generally retirees from the northern states, who come to the warmer climates to escape  the winter weather of their home states, usually between November and March. Harlingen has one mall, two medical hospitals and one mental health state facility, two libraries, one community college and numerous schools from primary to high school levels. Demographic and Epidemiological According to the website, County Health Rankings & Roadmaps, for Cameron County in 2014, teen pregnancy is measured by the teen birth rate per 1,000 female population, ages 15-19. The number for Cameron county teen births is 80, the number for Texas as a state is 57. There is poor quality of life in relation to health in Cameron County. 30% of the population has poor or fair health, compared to 18% in Texas. Cameron County also has an adult obesity rate of 27%, 21% of physical inactivity while there is 69% of access to exercise opportunities. 16% of the population suffers from excessive drinking and 396 counts of sexually transmitted infections. 36% of the population is uninsured, 86% graduated high school, 10.5% are unemployed and there are a whopping 48% of children in poverty, which has increased from the 2008-2012 numbers. The top causes of mortality in Texas are diseases of the heart, followed by Cancers and Respiratory diseases. Windshield survey During the observation of Harlingen for the Windshield Survey project, overweight residents were visible. The majority of the population is Hispanics, most were slightly overweight and appear relatively healthy. There are nursing homes and assisted living facilities in the community and they are mainly Hispanic and White older adults who are a mix of thin and frail to obese and unhealthy. Women are seen pushing strollers on the jogging trails, while the young children play in the parks. There are groups of teens who play football and basketball in the same area. In the residential areas, some people can be seen outside maintaining their properties, while other properties are dilapidated. The general appearance of the residents are healthy, with some being overweight. I have seen a few multimedia billboards for the local hospital, which is displaying an ad for bariatric surgery, which is appropriate for the current overweight population. Harlingen has some city parks and Boys and Girls clubs and numerous gyms in the area that are heavily utilized. The restaurants in the  area are mainly along the expressway and lining the few main streets around town. There is no food sold along the street, but people can be seen sitting in the parks eating meals. In Harlingen, the population is overweight Hispanics and Whites who have access to healthcare and wellness measures, as it is advertised constantly. There is adequate transportation and resources available for the population. Problem As stated before, Cameron County has an adult obesity rate of 27%, 21% of physical inactivity while there is 69% of access to exercise opportunities. While the averages for the state of Texas are 29%, 24% and 74%, respectively. Cameron County also has a limited access to healthy foods, 16% and 21% food insecurity and a mere 17% of children are eligible for free lunch. Since Harlingen and Cameron County have high obesity rates compared to the rest of the state, the Healthy People 2020 objectives applicable for Harlingen include: NWS-11 (Developmental) Prevent inappropriate weight gain in youth and adults and NWS-9 Eliminate very low food security among children. (Healthy People 2020, 2014) While these are two distinct objectives they are connected, much education will be need to be done to ensure change. The community needs to be educated and held accountable for their actions, at times they are unaware of how much damage an unhealthy diet can do. The Community Health Nurse must prepare for these challenges and have a good understanding of how to present information that will gain the interest of the population. A community health nurse must be well versed in the numerous efforts that are starting to prevent childhood obesity. There are new physical education requirements, nutrition standards for beverages and foods sold in school, to community initiatives to expand bike paths and improve recreational facilities (Koplan, Liverman &Kraak, 2005). â€Å"Families need to make their homes to more conducive to a healthful diet and daily physical activity (Koplan, Liverman &Kraak, 2005). The nurse will need to get out to the community via health fairs, school assemblies, media outlets, etc. to get the message out there. Education on making healthy choices, getting adequate exercise and making small adjustments to their daily lives will help. According to the article, Relative and Absolute Availability of Healthier Food and Beverage Alternatives Across  Communities in the United States, there was a study that was conducted on 3 years of data from the Bridging the Gap Community Obesity Measures Project. It stated the mean healthier food and beverage ratio was 0.71, meaning that stores averaged 29% fewer healthier than less healthy products. There were less availability of healthy alternatives in Hispanic communities (2014). Summary The community health nurse needs to be an expert in assessing and diagnosing health concerns in the community to be able to set realistic goals that will maintain the health of the population. Health promotion, education and community involvement are crucial. â€Å"Nutritional education is essential and must include parents, teachers, and the child† (Nies & McEwen, 2011). Compare/Contrast of Two and a Half Men

The popular television show, Two and a Half Men, has been on the air for just under a solid ten years. Even after ten years, and a complete main character change, it is still one of the most watched and beloved shows on television today. Most would argue, including myself, that the original version starring Charlie Sheen was much better than the current version, starring Ashton Kutcher.Both are very funny and have attempted to keep to the same â€Å"womanizing† main character, though the original pulled it off with much more success. The show was originally about a â€Å"pleasure-seeking† jingle writer, Charlie Harper, his uptight brother, Alan, and Alan's growing son, Jake. Charlie's laid back life becomes complicated when his brother gets divorced and moves in, along with Jake, to Charlie's beach-front Malibu house.The Harper brothers Charlie and Alan are almost opposites but form a great team for comedy. They have little in common except their dislike for their dull, emotionless, and dominant mother, Evelyn. Alan, a compulsively neat chiropractor and control-freak, is thrown out by his manipulative wife Judith who nevertheless gets him to pay for everything and do most jobs in the house. Charlie is a freelance jingle composer and irresistible bachelor who lives in a luxurious beach-house and rarely gets up before noon.Charlie â€Å"temporarily† allows Alan and his son Jake, a food-obsessed, lazy school kid who constantly moves between his parents, to move in with them after Alan's separation/divorce. The sitcom revolves around their conflicting lifestyles, raising Jake (who has the competent, caring dad while having a ball with his fun-loving uncle who teaches him the fun way to live), and bantering with Evelyn and various other friends and family.Other fairly regular characters include Charlie's cleaning lady Berta, who is the sarcastic and sharp-tongued character who merely does nothing more than insult almost every other character unf ortunate enough to cross her path (while completely ignoring her actual job), and his rich, self-confessed stalker neighbor Rose who often sneaks in to spy on Charlie and induce havoc into his already screwy life. Charlie's love life is a recurring theme in his character. He is an alcoholic womanizer who has engaged in decades of frequent one-night stands, prostitutes, casual sex, and â€Å"relationships† of short duration, in stark contrast to his rother Alan's inability to bring in much female attention. Even though Charlie usually never calls any of his partners again after he had sex with them, there are a few relationships on the show that lasted longer than one night, notably his neighbor Rose, who continues to stalk him after they spent one night together, and Jake's ballet teacher Mia, whom Charlie was actually in love with, and almost married in Las Vegas. He usually dated much younger women. Alan is almost the complete opposite of Charlie. He had been divorced twice over the series, and is notably seen as socially awkward with any woman he comes into contact with.Alan is essentially a leech on Charlie’s life, as he was only supposed to stay with Charlie for a short amount of time, that was ten seasons ago. Recently, though, Alan has managed to keep one person tied down and hold a steady, healthy relationship with. This person is Lyndsey MacElroy, who is actually a mother of one of Jake’s stoner friends. At the end of season eight, Charlie leaves in pursuit of Rose, who he admits he loves, to Paris. At the beginning of season nine, it is revealed that Charlie died in Paris, because he â€Å"fell† in front of a train (it is widely believed that Rose murdered him because he cheated on her, a lot).Enter: Walden Schmidt. Now begins the more recent version of the series. Charlie Harper is dead, and Alan and Berta must now move out of the fabulous beach-house and have no idea where to go. Schmidt is introduced to the series as a billionaire internet entrepreneur who has recently been divorced and is now suicidal. After unsuccessfully attempting suicide, he turns up at Charlie Harper's beach house and decides to buy it from Charlie's brother, Alan, to whom Charlie has left the house in his will, after dying in France. Alan has to put the house up for sale as he is unable to afford the mortgage payments and property tax.While Alan is speaking with Charlie's ashes and trying to decide where to spread them, Walden suddenly appears on the house's back deck, scaring Alan and causing him to drop the ashes on the living room floor. When Alan lets Walden in so he can use the telephone, Walden reveals he has just tried to commit suicide by drowning in the ocean. He tells Alan that he is worth $1. 3 billion, but would give it all up to reconcile with his wife Bridget, from whom he had recently separated. Walden and Alan then proceed to bond at the local bar, where Walden tells Alan that e made his money when Microsoft purchased his website. At the end of the episode Walden tells Alan that he is going to buy the house. This is fantastic news for Alan, as he now no longer has to move, Berta can be rehired, and a surrogate family has now been formed. Walden is a hopeless romantic and has had many failed relationships, ranging from divorce and breakups to rejection of marriage proposals. Walden's ex-wife describes him as â€Å"having the emotional maturity of a 12-year-old†, and she, his housekeeper Berta and his girlfriend Zoey have all described him as needy.However, he drives a Fisker Karma as he cares about the environment, and does not believe in lending money, as it is never repaid and leads to resentment of the person to whom the money is lent; instead he prefers to just give it away. He does not like the taste of alcohol and so does not drink much. When Walden met Alan Harper, the two almost instantly formed a friendship. Walden is very generous, which Alan uses to his advantage, such as when Alan's ex-wife Judith drops off their son Jake, and Alan convinces Walden to allow Jake to stay. Despite events such as this, Alan does demonstrate genuine concern for Walden.When Walden discovers that his imaginary childhood gorilla friend, â€Å"Magilla†, was real, and was part of an experiment being conducted by his mother, he becomes very upset, as he thought of Magilla as a brother. Alan climbs a roof to console him, telling him that he knows what it's like to lose a brother. Walden knows that Alan is poor and has offered him money, but Alan refuses because he wants Walden to see him as a friend, rather than a freeloader. From this point on Alan and Jake have fully moved in and Walden considers them family, refusing to kick Alan out of the beach-house, though knowing that he is quite the leech.While there has been much change to the popular television show over the years, it has managed to sustain a fresh comedy to most viewers and appears as if it could continu e airing for the next ten years also. The show is now very different, since the introduction of Ashton Kutcher replacing Charlie Sheen as the main character. But it continues to maintain the viewers, ratings, and new ideas that make a good show great. Comparatively, the older seasons are better than the newer, but the newer are certainly getting the job done.

In all Muslim areas, veiling and sex-segregation generally form part of the gender system.4 The seclusion of women from the rest of the world alongside the concurrent ban on sexualisation of women in the public eye has led to increased levels of sexual violence and abuse directed towards both women and children5. It is only recently that researchers have uncovered more accurate statistics on the occurrence of this abhorrent violence following the establishment of human rights based projects and refuges to rescue and protect women and children as well as to promote the welfare of the wider-public in general. 1) General Theoretical Background: Women and Gender An important foundation for the study of women and gender is to be found in the feminist literature of the 1970s. Around this time scientists had begun to untangle the complex interplay betwee n biological gender differences, and the abstract concept of gender, which is now a socially determined construct imposed on all human beings from the moment they are born. In this regard, the presence of transnational corporations (TNCs) and multinational companies (MNC s) has become one concrete witness testifying not only to the reality of the global market economy, but also to the actuality of a smaller world. Multinational corporations (MNCs) is generally understood as a company that has its headquarter in one country (which is the home country), and which operates in at least one foreign (host) country (Wilkins, 1991, p 53). MNCs, just like any other corporations, seek to increase profit via conducting business in another country as it aims to expand its market, acquire resources, to diversify sources of sales and supplies and to minimise competitive risk (Wilkins, 1991). Disease diagnoses that are most rampant in these home care patients are: Heart disease, diabetes, cerebral vascular disease, chronic obstructive pulmonary disease (COPD), malignant neoplasm, fractures, congestive heart failure, osteoarthritis and allied disorders, and hypertension(NCHS). Care from registered nurses, physical therapists, and social workers; access to medical equipments; check up by doctors; help with running errands; medication delivery; and meal preparation are some of the services that are offered by home health care providers. Types of home care vary with different kind of providers. Skilled care type is offered by licensed medical practitioners like doctors, registered nurses, medical social workers and physical therapist. This type of home care service focuses on the reinstating and maintaining the health of a client. It includes disease diagnoses, disease treatments, wound care, injection and intravenous therapy, medical assessments, nutritional management, and counseling. The other type of service is the home support services that are offered by caregivers or health care aides which are more inclined to domestic tasks like personal hygiene or bathing assistance; running errands; grocery shopping; cooking and eating assistance; light house keeping; and medication reminders(MFMER ). Clients of home care services expect the providers of these services to meet their specific needs. The most common client specifications are good quality care; availability of service; health service provider with appropriate training and expertise; high quality, effective care; and affordable service. Home care providers are expected by clients to give them respect, inflict no harm to them, and treat them well(Nina Preto). They also expect to have their rights as a patient considered, respected and instituted. The law currently implements the home care patients right to: choose their care providers; be informed fully by the health service agency provider of his/her rights and responsibilities as a patient; obtain the needed professional care following the doctor’s orders; receive continuous care; be asked consent prior to any treatment regimen or procedure; advised for any plan changes prior the change; avail service that is safe and in accordance to the professional care needed; be informed of actions in the event of emergency; and to voice opinions regarding the treatment or procedure(Hospice). Different individual conditions require varied specific needs so the provider should be competent enough to be able to address these circumstances. The geriatric or elderly persons for example have various conditions that have complex needs. Usually diseases of the old individuals are multi-factorial; the change of their condition is unpredictable; and outcome of treatment or management is random. So it is best if the home care providers of the elderly with diseases to be registered nurses or doctors for prompt treatment and disease management. Though in other instances home care is employed by geriatric patients that are healthy but in need of assistance in performing their daily tasks from as simple as opening doors for them to household chores. Another example of patients with different needs is the patients with cancer. These individuals are more at ease in the confines of their homes and families. They do not want to be separated from there families because usually these patients gain there strengths from them. The emotional and moral support the families give are vital for a cancer patient. As cancer is a devastating disease the home care provider should be able to give support to the patient, address his/her medical needs, and educate the family about the situation of the patient. Diabetes patients on the other hand, need home care providers to always encourage them to control the blood glucose levels because complication development can better be prevented with serious control on glycemia. These patients also need home care providers that are inclined in nutrition for diabetes patients to obtain the needed nutrients without sacrificing their blood glucose levels. Along with the boost of home care industry are problems or issues that need to be resolved. Some of these problems that affect both the home care consumer and the provider are the following: worker related injuries; worker abuse and exploitation by the client; competency of the health care provider; abuse of the client; client prejudice toward the caregiver; worker benefits issue; confidentiality issues in the clients home; and clients being at risk of theft and other forms of crimes. Confidentiality issue in client-caregiver relationship is one of the prime problems of home care services. Prone to this problem are the caregivers or health care aides that have little backgrounds or training in professional ethics and usually provide service to their clients twenty-four hours a day, seven days a week. Most of them also stay in the houses of their clients so it is common that they know the things that happen inside the house especially among family members. The dilemma is in situations that either the caregiver spill the information he/she knows or if the information is critical to the well-being of the client and the caregiver will not know whether to keep it confidential or not. An example situation is when a client is being battered by a member of his or her family, what will the caregiver do? Is he/she supposed to tell anyone like the authorities or is he/she will just keep silent and wait that other people will discover the situation? If the caregiver gives the information to authorities it might be an invasion of the client’s privacy. Moral risks, virtues, intimacy, and professional ethics are bound with confidentiality issue in home care. Home care service providers should be made sure to have proper training in professional conduct and ethics to avoid any problems that will interfere with the client and their family relationship. Problems such as this needs attention of legislators for the home care consumers to be secured that the service provider they employ are properly trained and knows his place as a home care worker in a family setting. Client prevention of problems related to the home care service provider begins with choosing a competent agency that ensures the capabilities of their workers. Clients should not just consider the monetary factor involve in employing a home care worker to prevent the hiring of low paid providers and yet incompetent in many aspects of health service provision. Health care organizations have to address these problems and anticipate other issues that will emerge for the improvement of the home care service, benefit of the client, and avoidance of dilemma in the part of the caregiver. Daisy consistently had the courage to stand up to the other Americans, the characters who adapted to the European culture, even though they continued to pressure her, and prevailed by not conforming to European society. Daisy is a hero in this story because of the way she approached and handled situations. A major conflict in the story is the clash of American culture and European culture. Daisy is constantly thrown into situations where she is pressured to conform to some traditional European way of doing things. Daisy does not let this pressure get to her. She instead continues to act without fear of what might happen to her. The text shows many examples of this. A major cultural conflict that happened in the novel is when Daisy is walking around the Pinchio with Mr. Winterbourne and Mr. Giovanelli. Mrs. Walker comes up in her carriage and begs Daisy to get in because of what her reputation might become since she was walking around with two men ( ). After Daisy refuses, Mrs. Walker says â€Å"should you prefer being thought a very reckless girls? and even gets Mr Winterbourne involved by having him give his opinion which lined up with Mrs. Walker’s( ). After examining the text, the reader sees that Daisy responds with a very fearless and courageous way, which in a way can also be compared to a quote from Emerson’s Self Reliance, â€Å"to be great, is to be misunderstood. † â€Å"â€Å"If this is improper, Mrs Walker,† she persued, â€Å"then I am all improper, and you must give me up†Ã¢â‚¬  is a quote that beautifully illustrates Daisy’s heroic personally by not being intimidated from the pressures of society. Another conflict in the novel was that at certain places at certain times, the chances of getting malaria were very high. While many of the locals decided to stay away from these areas, Daisy chose to not live fear. This fearlessness that Daisy displayed, led to Mr. Winterbourne confronting her and Mr. Giovanelli at the coliseum late at night about her catching the Roman fever (). Giovanelli responded to Winterbourne by saying he informed Daisy it was a â€Å"grave indiscretion† which Daisy responded with prudence (). Even now that Daisy was severely warned about the disease she still showed no fear and showed how she could be an inspiration to other women by saying she had a lovely time seeing the coliseum by moonlight and if she felt any danger she would take some pills (). â€Å"I don’t care,† said Daisy in a little strange tone, â€Å"whether I have Roman fever or not† shows how Daisy would not let fear dictate her life( ). Daisy Miller was a hero because of the fearless qualities that she displayed in the novel Daisy Miller. She may not display the common stereotypes of other fictionalized heroes such as flying or x-ray vision, but she does have influence on people, especially women, on how they can act regardless of what culture they are from. This view on life is very important because it shows young women, that they are empowered to handle situations anyway they like. Disney is famous for his influence in the field of entertainment during the twentieth century. As the co-founder (with his brother Roy O. Disney) of Walt Disney Productions, Disney became one of the best-known motion picture producers in the world. The corporation he co-founded, now known as The Walt Disney Company, today has annual revenues of approximately U.S. $35 billion. There are several means in which the controllable factors could be avoided in other to present Euro Disney the success it needed at the first year of business. This means are suggest and proposed as follows: The deliverance of advertising to the customers, not on the physical composition of just the area alone, but on the resources and service that a customer will get from patronising the resort. Advertising should be careful controlled. Society needs to make sure that people are not taken in by misleading advertisement. There is a set of principles that all advertisers must follow (Roman Mss, 2003, p.200): Tell the truth, show the truth- the product should look exactly the same way as the consumers will purchase it Make the general impression truthful- Advertising is judged not by what it says, but what consumers thinks it says. This brings the world together. The EuroDisney could use the help of the Disney in Turkey in other to be able to handle the cultural difference in French. By understanding the French culture, EuroDisney will be able to bring polices that will be accepted by the French customers. The French cherish their culture that it is important for EuroDisney to able to adapt the culture and avoid cultural self criterion, rather try to understand the culture of the French. A company reputation and size of Disney is allowed no room for mistakes. The stakes involved are billions of dollars. Complementary businesses like that of the hotel industry are reliant upon the success on the success of this team park in Paris. Generous funds received from the government and private institutions would have to be made well of. Disney should be able to foreseen the unforeseen. When Euro Disney was established the consideration taking into account is the geographical area and culture as well. However Disney established the Euro Disney based on the nature or American thinking, they expected the Europeans to act as Americans and thrive over newly designed theme park. They forgot that they were producing a massive theme park bigger than any other theme park in Europe for the American mentality. Retirees over the age of sixty two in the United States are estimated by an independent financial survey to be sitting on over one trillion dollars in total assets. That money mostly lies in financial institutions gathering interest while needy family members suffer impatiently waiting for the day when it will be their inheritance. But not all seniors are that callous, they give away their money to loved ones before they die. There is no better way for the elderly to show that they care. Those that do can enjoy the pleasure of watching their money being spent. It is a joy that they would never experience if they waited until their death. The country is in a recession that could be ended by an influx of spending money. The economic stimulus money that will be received by tax payers during the summer months will help but it is not enough. More money is needed. The economy would improve and the recession ended if the trillion dollars held by seniors was spent. It is up to them to end the recession in a loving way. Immediate handing over of their unneeded assets to loved ones would cause a spending spree the nation has never witnessed before. It would put the unemployed back to work and end home foreclosures. I am sure if our elderly realized their power to help they would quickly respond. I had a close friend who suffered with a nervous disorder all of his life. He lived with the help of a government welfare program which just gave him enough to survive. His parents were millionaires in the real estate business and owned at least twenty apartment houses. They let their son live rent free in one of them. But they gave him nothing more. I spoke with him many times over the years and listened to his complaints. He had little to eat and was lonely. The only hope he had was that one day his parents would die and leave him with their millions. They both were dead when he reached the age of seventy two but the money was left to his two sisters and their children. All he inherited was the right to continue living rent free. I never witnessed such cruelty. Many seniors are sitting on more money than they need while family members no longer living with them are destitute or have financial problems.

His short story â€Å"Children of the City† is a departure from his usual style. It won the PalancaAwards in 1971. Setting This story happened in the late 1980’s. Everything occurredin the dark perilous busy streets of Manila. Boulevard . streets of avenida Characterization -Victor He is a boy from Intramuros. At the age of eight, he loses hisfather and his mother abandons him for some other man. He ishired as a newsboy by his uncle and starts his life on the streets.Innocent and young, he ponders on the menace and vices of hiscolleagues and the people all around him. -Victor’s Dad A good-natured man and a loving father to Victor, he was apart of a worker’s strike. The man loved his son dearly and hisdeath brought a huge blow to Victor. -Victor’s Mom She loved Victor’s dad but never cared felt any affection forher son. She took her husband’s death grievously. But later on,she ignores Victor, becomes a prostitute, then leaves Victor to hisUncle to go away with her new lover. -Tio Pedring Victor’s uncle, he adopted victor after the leaving of hissister. He forces victor to become a newspaper boy. -Nacio He is a newspaper boy. He was victor’s new friend. Thoughfull of cruel vices, Victor took his death as another major loss. Summary The father of the boy Victor got involved in a strike. He actsas though it was nothing and takes Victor to night walks aroundManila. He takes Victor’s mind off vices and promises Victor abright future someday.One day, during the strike. Victor’s father was shot dead inthe heart. His wife mourned greatly and his son was subdued. Thewife began to disappear late at night and come home early dawn.She refuses to take care of Victor. And then, she comes homewith some goon, telling that the goon will be Victor’s new dad.Victor didn’t like the stranger at all. As often as possible, he triesto stay out of his mom and her lover’s way.And then, Victor just woke up with his mother and the mangone. His Uncle Pedring introduces to take care of him. Herecruits Victor to a newspaper job. The man does not treat Victorwell unless he brings home money.During his job, Victor meets Nacio. Nacio was also anewspaper boy. He taught Victor various tricks in newspaperselling. They become close friends.Victor’s job grew prosperous. Soon, his â€Å"Boss† starts to trusthim with a ration of 20 papers a day. He becomes contented withhis life until-Nacio was run over a car. He was dead. Victor grieved for hisfriend just like the way he did with his father.
